How do I create a questionnaire template?

Create questionnaire templates and add them to a session type so clients can easily share important information with you right as they book their session. 

Questionnaires are part of our Booking add-on and are included in all booking plans. 

Create a Questionnaire Template

  1. Go to Studio > Questionnaires > New Template (top right corner).
     
  2. Name the questionnaire. This name is internal, so name it something that is informative to you.
     

  3. Optionally add a description. This displays at the top of every page of the questionnaire, just below the client info. Share some details about the session or the questionnaire itself here. 
     

  4. Click the Add Question button to add a new question. Select the type of question you'd like to add:
     
    • Short Answer - For freeform answers up to 500 characters
    • Paragraph - For freeform answers up to 3,000 characters
    • Dropdown - Add answer options that show in a drop-down menu
    • Radio Buttons - Add answer options that show as radio buttons

    • Checkboxes - Add answer options that show as checkboxes and allow clients to choose multiple selections

      You can toggle on Required next to a question to ensure that the client has to answer the question before going to the next section or submitting the questionnaire. 

      For short answer and paragraph questions, optionally add placeholder text to give sample answers or add more context to the question. 

      Drag and drop questions to reorder them by grabbing the stacked dots icon to the left of the question. For questions with multiple answer choices, you can also drag and drop the answers into the order you'd like them to show.

  5. Optionally add additional pages by clicking Add Page.
     
  6. Click the Themes tab to apply one of your existing themes or make a custom theme for this template. 
     
  7. Click Save

Start from a Pre-Made Template

We worked with our ShootProof Community Group to create questionnaire templates made of common questions frequently used by our photographers. 

To make a template from a pre-made template, Click Create a Template to make a copy of the template that you can then edit.

 

Add a Question Template to a Session Type

Add a questionnaire template to your session types so you can easily get info from your clients as they book their session. 

  1. Go to Studio > Booking
     
  2. Click into a session type
     
  3. Go to the Rules tab
     
  4. Select the questionnaire template from the Booking Questionnaire drop-down menu
